Odontogenic Sinusitis: An Interdisciplinary Approach to Diagnosis and Treatment

Category: Implantology

Instructor(s): Inna Funtikova

Course structure: Lessons: 4 lessons | Duration: 1 h 50 min

Course overview

Odontogenic sinusitis represents one of the most complex pathologies at the intersection of dentistry and otorhinolaryngology. Successful treatment of such patients requires not only accurate diagnosis but also a deep understanding of the mechanisms of disease development, modern imaging capabilities, and effective interdisciplinary collaboration.

This course is dedicated to a comprehensive interdisciplinary approach to the diagnosis and treatment of odontogenic sinusitis.

During the course you will study:

The etiology, pathogenesis, and clinical manifestations of odontogenic sinusitis, as well as current principles of its diagnosis
The capabilities of CBCT, MSCT of the paranasal sinuses, and endoscopy when examining patients with maxillary sinus pathology
Algorithms for choosing conservative and surgical treatment tactics depending on the clinical situation
Principles of interdisciplinary collaboration between the dentist and the otorhinolaryngologist in managing patients with odontogenic sinusitis
Modern approaches to the surgical treatment of maxillary sinus pathologies, preoperative assessment before sinus lift, and prevention of implantology complications.

Course lessons

Lesson 1. Odontogenic sinusitis: etiology, pathogenesis and clinical manifestations

Main causes of odontogenic sinusitis development: periapical infection, periodontal pathology, complications of dental procedures and rare etiological factors
Mechanisms of infection spread from the oral cavity to the maxillary sinus and features of the disease pathogenesis
Clinical manifestations of odontogenic sinusitis: main symptoms, signs and features of disease course
Differential diagnosis: distinguishing reactive thickening of the maxillary sinus mucosa from a true inflammatory process
The role of dental factors in pathology development and the importance of comprehensive assessment of the condition of teeth, periodontal tissues and the sinus.

Lesson 2. Diagnostic criteria and imaging: CBCT, MSCT of paranasal sinuses and endoscopy

Diagnostic algorithm when odontogenic sinusitis is suspected
Clinical signs of the disease, capabilities of modern radiodiagnostics and nasal cavity endoscopy
Assessment of the natural ostium, the ostiomeatal complex, the extent of inflammation and indications for otorhinolaryngologist consultation.

Lesson 3. Management of patients with odontogenic sinusitis: conservative therapy, surgical indications and interdisciplinary treatment algorithm

Modern approaches to managing patients with odontogenic sinusitis at different stages of the disease
Indications for dynamic observation, conservative therapy and surgical treatment
Criteria for the necessity of joint management by a dentist and an otorhinolaryngologist
The role of elimination of the odontogenic focus in removing the cause of the inflammatory process
Reasons why pharmacological therapy cannot replace removal of the primary source of infection
Principles for constructing an interdisciplinary treatment algorithm to achieve a predictable outcome.

Lesson 4. Surgical treatment of maxillary sinus pathologies and preoperative assessment before sinus lift

Main surgical options for treating maxillary sinus pathologies in otorhinolaryngology practice
Endoscopic surgical access methods: antrostomy via the middle nasal meatus, access via the inferior nasal meatus and prelacrimal access
Capabilities and limitations of external surgical approaches in maxillary sinus diseases
Preoperative assessment of the paranasal sinuses before performing a sinus lift
Contraindications to performing a sinus lift related to maxillary sinus pathology
Criteria for interdisciplinary interaction between the dentist and the otorhinolaryngologist to increase the predictability of implant treatment.
